OIG Provider Sanctions

Please use the Search below to identify businesses and individuals by name. We advise that you do not take any action on a provider until you have verified their status
with the HFS/OIG. Inquiries should be directed to the hfs.oigwebmaster@illinois.gov.

You may filter the Sanction listing as follows:

• Searching for specific words or partial text. To Search on partial text, use * before and after the search value. (Ex: *Oak*)


• Selecting Active Sanctions or Recent Additions using a dropdown.


• Selecting a specific Action Type using a dropdown.


• Export Data saves the records displayed to an Excel spreadsheet.


• Always press Reset before filtering a new list to Export.
